The School Nurses are responsible for promoting optimal levels of health and well-being within our school community. We collaborate with a student’s family, support their medical home, and provide care and compassion when a student is ill or injured.
We recognize that a healthy child learns better.  This is accomplished through the ongoing appraisal, promotion, and protection of student health and well-being:

  • Appraisal: Health assessment, evaluations and screening
  • Promotion: Health instruction, advocating for student health, community education
  • Protection: Immunization compliance, tracking communicable diseases, promoting safety

 

School Health Forms

Please use Final Forms to complete and update your child’s health information. A copy of your child’s current immunization record is required per ORC 3313.67 and can be uploaded into Final Forms. 

Vision and Hearing Screenings

 

Vision is routinely screened in Preschool, K, 1, 3, 5, 7 and 9 (See Orange Schools’ Calendar for dates of screenings)

Hearing is routinely screened in Preschool, K, 1, 3, 5, 9 and 11 (See Orange Schools’ Calendar for dates of screenings)
